One month is sufficient time I believe. I think you can apply as early as 3 months in advance. My visa was issued the next day after my interview. But you usually will know if your visa application is successful immediately after the interview.
As long as you're still in status, she's still in status. She may have some more explaining to do at the point of entry as to why she didn't come into the US immediately though. But I don't see this as problem. However, you can always double check with the issuing embassy.
Not sure what to make of this, however I do not think your school is right for refusing to change your I-20. Yes, some fraudulent people take advantage of this however, this may not be the case. I also know many people who have succeeded in doing this, and have legal dependents (not a fraud).
To be honest, it will be very hard. But just be honest with the Consular Officer, and properly explain what happened and how it will be different if granted a visa for the 2nd time. You really have no choice here.

There is nothing wrong with emigrating for school although you have a partner in the US and if the Consular Officer denied his visa, it probably won't be because of that but for other reasons. But go ahead and apply again.

You cannot come into the US without a valid I-20, the I-20 is more important than the visa - your visa can expire, but as long as your I-20 is valid, you're good to be in the US (can't travel).
You don't need a new visa, it's the I-20 that will be changed to reflect your new university...

I don't know if that's how it works for B-2 visas. But with an F-1 visa, as long as your I-20 is still valid and you're in school, you can remain in the country. When you receive the letter, you can always apply again. I suggest you wait till you receive the letter.
