VIDASH,
Here's what happened in my case. This was for Mumbai consulate.
I did not get medical report prior to interview date. Still I went for the interview and they collected all papers and took signature and asked me to come back with medical report any time and collect visa.
I got...
SSN for dependents
In the past Social Security Administration has been changing rules for issuing SSN number for H4 applicants.
I know number of people who have SSN with H4 status.
I don't know what's current rule.
Mumbai Interview : 7/9
Date of Entry : 7/11 SFO
Welcome Letters: 7/30 (2 Copies each)
Plastic Card: 8/1
SSN : Awaiting (Elected to get new cards on DS230-II)
Welcome Letters and Plastic Cards both issued from
INS California Service Center
Mailed from Laguna Niguel, CA
Answer to your 2nd question.
If u switch to CP.
Your faimily can stay back as long as they maintain legal status. Any of the dependent members will have to come to your home country for interview process till than they can stay in US.
I agree with u. Now a days for CP cases one should expect welcome letter/card by 3rd/4th week.
I entered from SFO. No problem at all. It was as easy as one could think of. Got welcome letter today (its 3rd week).
chiks,
Look at http://www.immigrationportal.com/showthread.php?s=&threadid=78010

In your case u need it for your wife. U don't need one for the daughter. U just need to carry photo copy of your daughter's birth certificate and passport.
FYI.
In my case I never received PKt 4. This was for Mumbai Consulate. Don't know what happened to it.
So would advise do not wait for it. Once u know from the web site, u are scheduled for a specific date, one should ask some one in Bombay to collect it.
When u ask some one to collect...

Question No. 3: Should I fill my name here?

This was with respect to Police Clearance for foreign country (Other than US and India in this case).
