Transit Visa Requirement?

I gathered the following information on transit visa requirement from various threads under CP sub-forum:

Transit Visa:
* Is required for folks who have an expired I-94 and expired visa stamp in the passport but still have the new visa approval (I-94) papers. - Is this correct?

* Is required if one takes a connecting flight from: Frankfurt - Germany, Paris - France, Zurich - Switzerland. Are there more cities - countries?

* If one wants to avoid especially the CP'ers who come to know about their CP interview last moment, it may be better to fly from into London - UK, Amsterdam - Netherlands, Milan - Italy, Singapore City - Singapore and Moscow - Russia. Is this list accurate? Are there other options?

Please respond so that all potential CP'ers can find transit visa answers in this one thread!

One can also go via Seoul - South Korea. (I have travelled with expired I-94).
My guess is Hongkong also does not need any transit visa.
