Here is the complied list of all what you required to provide for your 485 process!
1. Each applicant must have an original long form birth document. A copy must be submitted to our office for filing with the Immigration Service. If the document is not in English, it must be accompanied by a certified translation.
2. Original marriage document. A copy must be submitted to our office for filing with the Immigration Service. If the document is not in English, it must be accompanied by a certified translation.
3. If previously married, you must furnish evidence of termination of marriage (Divorce Decree or Death Certificate).
4. Each applicant must have two photographs for each application filed. Therefore, you will need six photos taken. A photo specification sheet will be sent to you with your 485 documents.
5. A letter from your bank showing when the account was opened, type of account, and the current balance.
6. A letter from your employer. We will provide you with this letter in your 485 documents package.
7. U.S. Income Tax Returns for the past three (3) years.
8. Medical Exam. A physician's list will be enclosed with your 485 documents package. Each applicant must have a completed medical to file with the Immigration Service. Please contact a physician on the list and schedule your exam. The physician's office will give you a SEALED ENVELOPE - DO NOT OPEN THIS ENVELOPE. THE MEDICAL MUST ALSO INCLUDE THE VACCINATION SUPPLEMENT. THIS WILL BE PROVIDED TO YOU AS WELL.

10. Passports. Please copy each passport in its entirety from back to front, including blank pages. All passports covering your entire stay will be required. If you do not have copies of all passports, other arrangements will be made.
11. The Immigration Service requires that we prove you have been in proper status since your first entry. For example, we need copies of all transcripts and I-20s if you were a student, all H-1 approval notices if you worked in that status, and any other immigration documents you may have.
PLEASE REMEMBER YOU MUST CONTINUE TO MAINTAIN WORK AUTHORIZATION AT ALL TIMES UNTIL YOU ARE ACTUALLY GRANTED PERMANENT RESIDENCE.
