I thought the same but it seems there are (few) cases where a pending name check can keep everything on hold.
I can only aproximate that the name check was cleared mid September.
When I took an infopass appointment to get the interim EAD I found out that all applications were onhold due to name check. I am from Eastern Europe and my name is not common. I did nothing to clear the name check other than to inquiry when the applications were outside processing dates.
At the beginning of the November I called USCIS to inquiry about the status of my pending I-485. I've got the following response:
"Your application is awaiting assignment to an Officer. You will be notified when a decision has been made or if additional information is needed."
Couple weeks...
Finally approved!!!
After a wait of more than 8 months I received the email notifications that my long pending I140 has been approved.

Many people get RFE for documents they already sent along with the applications. Earlier in this thread I asked the same question and it seems that it is easier for USCIS to ask for documents or maybe they simply hope you lost your approval.
I was exactly in the same situation and my lawyer told me that there is no problem to file I-485 while H1B extension is still pending. Worst case scenario you will get a RFE to show that your extension is approved.
Do not waste time and file I-485.
The fast approvals as TSC were for applications with PD not current. The target clients for premium processing are those with PD current. There is no reason to pay extra to get an I-140 approval and after that to wait a few years to apply for I-485.

I would be surprised to see approvals with RD in Feb or Mar now that USCIS wants to implement premium processing services for I-140. 3-4 month backlog will be the norm. Look at I-129, it is constantly in a 3 month backlog even when H1B quota is exhausted.
