    Don't worry about disclosing or bringing documents. This is a minor traffic violation, fined under $500 and not DUI related. You don't have to mention it during the interview either. If the IO asks, which I doubt he/she will, just mention briefly.

    Drivers License Address not current

    My DL also has an old address, although I received a card from DMV with the new address. I went to my interview last week, and when I was about to give that DMV card, the IO said no need. He just wanted to see the DL (with the old address).
