· Form I-765

· 8 visa photos (2X2” size, with white background)

· Valid passport

· Medical from Panel physician. (V92 physical medical)
Original and copy of birth certificate

· Original and copy of marriage certificate (if any)

· Original and copy of Urdu Nikah Nama with English translation (if any)

· Proof of relationship between Petitioner and applicant

Bro sorry for asking many questions, why did they requested urdu nikah nama ?
 
Sorry to say, brother I requested you so many times to please read the thread carefully and avoid such type of questions which causes the waste to time. Everybody including me is telling you in previous thread that embassy ask to bring the marriage certificate. So think what is marriage certificate? It’s nikah nama. So again please don’t mind and I request you to please avoid wasting time on asking questions already explained. Thank you

No need to say sorry brother, i just wanted to make sure about @crusty boy last thread, have a look on his post the embassy separately requested him to bring marriage certificate and nikah nama, take it easy bro and make sure what you have posted.

You need to put your information in there. They need beneficiaries information now, because your petitioner already did all these forms when filing for asylum.

That form is to get the information about beneficiaries. If there is any column which asks to proved any information about you, you can put it there. Otherwise, mainly that is for beneficiary.

Actually i meant this bro, i just have marriage certificate along with English translation, it would be enough!
