I lost my passport last year and I-94 too with it. I did not face any probem in PP stamping.
Just tell them that that is lost, they don't care for that. They stamped our PP without giving any hard time, no question asked.
Persons having Green Card / Stamped Indian Passport / US Passport do not need any VISA to go to Canada !!
Should keep Passport with your Green card at the time of Canada Visit !!

In my case last year, the status did not changed to CARD mailed after changing to CARD ordered , even though I got the CARD in two weeks after that.
The feedback I received from Lawyer that once a person starts working on EAD then he/she can not use H4 status.
I don't think that getting SSN should not make any difference to H4 status !!
Yes, you need to have VISA to pass thru London if you do not have Valid visa stamped on your passport or GC !!

That is the reason, they got their mail returned !! Call your lawyer and ask them to update their address at CIS.
I do not think it will affect you relating to your physical card but why to take chances if you could make the things straight !!

It is normal, all the short validity passport is extended in the same way ! Our passports are also extended exactly like this and
we are planning to visit India this summer !
They do write a little note below the expiry on first page to see page # .
