TN visa by mail - Change of employer

I had earlier 2,3 TN visa with same employer and now applying for new TN visa by mail. I have questions -

1. Who will file I-129 (new employer or can I file myself)

2. Which service center the packet should go Vermont or California; as Vermont deals with only extension of stay or change of status for NAFTA and my case is for same status TN with Change of employer?

3. Should I file I-539 (dependents) after receiving this approval or now along with I-129 is better?

As far as I know VSC takes ALL TN applications. It is an extension of status -- TN status.
