Could you clarify what case approval? You didn't mean the asylum approval, did you?
Like when was your asylum case approve and what is your GC date?

I just got a response from CIS Ombudsman that my case was transferred to another field office (which is in different state then where I live) from NBC but my online account does not REFLECT any update. what does that mean?

I think NBC sends to a field office that has available officers to take on cases so I think you would be hearing from them soon.

Initial evidence means i-693 medical but additional i guess it will be something related to your asylum case ! Good luck and contact your attorney
it is a medical request if you haven't sent it with the application.

I guess as i know and heard if it’s because of medicals then they will mention initial evidence not additional evidence! Additional evidence is different !!!
That’s what it shows when you get RFE for medicals

I did sent it but it has been more then 2 years..
same here i also sent my medical in Nov 2020 i guess they will want another one...

Most likely it’s the medical cause you already submitted it before so it says additional this time cause you already submitted the initial one but it’s expired so now they need additional new medical report hmm i guess that’s it! Good luck but hopefully we all get approved quickly after the RFE was received as i got this status one week ago but nothing changed since then and it’s becoming weird that they don’t approve the i-485 immediately after getting the medical report!! i mean they already looked at the i-485 and saw that it’s all good and no interview needed but they just needed the medical report to approve it and still make us wait!! USCIS will always be USCIS no matter what!
