Sharing Some Research Result on G-731N
Since nobody replied, I did some 2-hour thorough research through this portal and the web. I could not find the answer to item 3 and 5 (item 5 - Date Entry of Adjustment, in particular). But I believe (and agreed by some posting) that only page 2 is needed.
After I almost gave up with item 5, I went to the INS official Chinese version of the form and found out what the heck "entry" means. Chinese clearly indicates item 5 is "Date of data entry for adjustment of status". To reconfirm, I opened up the Spanish version and with the help of a friend, it means "date of immigration status change". So here entry is NOT entry of person, but is entry of data. To this end, it is simply the date of adjustment of status, or the date of passport stamping.
To summarize,
1. Only sheet 2 needed.
2. For item 3, either port of last entry or office of passport stamping (for me they are the same). But the latter makes more sense.
3. Item 5 = Simply the date of passport stamping.
