No need to notarize the FOIPA request, as long as you send it with your original signature. They no longer accept faxed requests, due to the real signature requirement.

Just the completed form is enough. You're just exercising your right provided under the FOIPA act, so no explanation is necessary.
 
I got my NOA today. the post mark is November 30th
