For the DV lottery you should use the I-134 form (this is mentioned in the DV-2010 instructions on the travel.state.gov website).

I can't imagine that it will be a problem for the Immigration since your visa is still valid. But it is up to you to decide whether you feel comfortable with it. You will not have much space to manoeuvre in case you cannot travel on the day you intended.
