I second what TC had said. Your case is flagged. Getting an extension is difficult and you will face lot of problems when you comes next. This had happened to my friend.
Will it not be easier for the OP to visit his nearest CBP office and try his luck in a day rather than applying for an extension and wait for 2 to 3 months. If the CBP office says no then he can apply for an extension. In my opinion trial costs nothing.
Go to the nearest CBP office and contact deferred inspedtion section. Explain the problem. They will issue your MIL a new I94 with the same date as your FIL
Be assured that she can stay in USA till the last date mentioned in I 94 However, as a precaution, she should leave USA atleast one week prior to the expiry of I 94 date. She can apply for a visitor visa once she reaches back her home country.
Most likely they will forward to the correct cntre and the decision will be delayed. Alternatively they will return to you to forward to the correct centre. Please wait and watch.
i just returned a month ago after staying within my extended date of stay. I gave at POE the right side portion where no date was mentioned along with the old I 94 card and retained the left side portion which says " This half for personal records"

The extended I 94 form contains the sane number as that of the original form. If the number is clicked it will also give the lattest I 94 validity of stay. As TC suggested it is good to carry the extension approval letter also.
If there is an emergency situation which warrents your stay further than your I 94 expiry date, then, you can apply within 6 to 8 weeks of I 94 date. My friend did this and got an extension of stay.
She need not worry on this because she had already surrendered her I 94 form. Ask her to retain her boarding pass/airticket and bring the same when she comes again. At POE if asked she can produce the same as a proof that she left USA well before I 94 date.
