We just did IEAD for my wife yesterday. I can confirm that the count starts from your RD. RD would be 0, the next day is counted as day 1 and the earlist date to apply IEAD is the date when you count 91st. Hope it helps.
Usually the local office has devices to take the digital photo and finger prints, you will be asked to take the picture there. However, I suggest you bring two photos in case they don't have the device. There is no harm that you have the photos but not use them.
