Have you done your FPs? I think they can enter both names at Fingerprint time and new name can go under other names while app can still be under old name. Check out http://www.immipros.com which has answers to tons of N400 related questions and also has sample tests.

I am not sure...but this is what I would do if I were in your shoes.. I would use the new name...and take both passport and marriage certificate to my I-130 interview. At most they would advise her to update the docs with new name.

Of Course You will needto submit your medicals with I-485; although you can submit I-485 without medicals and then respond to the RFE later.
