i was keep filing extension every 6 months but just received letter staying that we will notify you and but to be honest i was not very much aware about the rules.
i dont have copy of i-94, i never get anything saying your extension is accepted but i do have the letter statying that we received your request (the standard letter)
if i go to check the status of my mother online (https://egov.uscis.gov) it still says that we will notify you when we take action.
i was looking at the paperwork i found one i-94 i guess it was very first request for extension. but answer to your question yes... again i was not very much aware of the rules and its my mistake.
My guess is that she overstayed by over 365 days (which makes sense, because AFAIK a B cannot be extended beyond 1 year) and the 10-year bar kicked in once she left the US.
