I have received a letter from Dept of Justice (Fbi) after my inquiry about name check, they have send a form to fill it and get it notorised and send it to FBi so i can know about the status of my name check, is it normal.
thanks for your reply.

This hasn't been reported at all from what I have seen in the forums. Can you post the form number (ex: N400 or I130) so others here can see the blank template of the form?

With whom did you make the name check inquiry? The FBI directly or through a congressman?

i wrote to laura bush regarding my N-400 and was stuck in name check. then i received this letter from dept of justice and a form with reference number so i sign and notorised the letter to FBI and they have send a copy to Mr. cannon, this is regarding name check status i was not even fingerprinted because they told me that they have my fingerprint from I-751.

Fingerprint expires in about 15 months. I have seen several cases which USCIS used recent fingerprint result for a different case (same person) for Citizenship purposes.

I am going to send my request to record mgt dept of FBI, and they promised me that they will promtly reply me. lets see wat happens, and the form has to be nortorised.

I also wrote to Laura Bush and received a reply from Michael Cannon's office, stating when my background check was initiated and when it was completed. This is the first time I'm hearing anything about a notarized form.

Interesting. Hopefully a good sign but more likely just more bureaucracy - by the time you get anything the whole thing may be done.

Bonsymth
the form you send is alost the same except it ask me about alien number on it
and a reference number aloted to me
thanks

I forgot to mention that, with every inquiry letter that I sent to various politicians, I included a signed statement authorizing them to obtain case information on my behalf. If you haven't included a similar statement with your correspondence, chances are they sent you this form for that exact reason.

If I'm not mistaken, most politicians request that release before making inquiries on your behalf.

You're absolutely correct. Some politicians have a release form on their website, others request that a self-prepared release statement is included with the assistance request letter. However, neither Dick Cheney's nor Laura Bush's websites mentioned anything about the release forms/statements. I just thought that it would be good practice to include those.
