Receipt/EAD Tracker DECEMBER - 2001!!!!***

Received receipts on December 24
Mailed on 12/06/01 (three AOSs and two EADs)

Our A#s are successive but EAC#s are not.
BTW, we paid $240 total for medical forms (family of three). They did the physical check and took the blood sample but sent us to our primary care hospital for the X-ray and shots (they donot have this).
We were not charged for the X-ray and shots--May be the insurance company took care of it.
