Questions for EAD renewal?

1) Do we need to send I-94?
2) All pages of passport or just first page?

I am planning to apply online.

Both are not needed for renewal. old EAD copy or mailer, I485 receipt copy are all that are required as support documentation.

USCIS fees $120
Copy of old EAD (front and back)
Receipt of I-485
Photographs (write name, DOB, and A# back of photo with pencil)
Top portion that you received at the time of Earlier EAD
*** If applying for spouse better also attach marriage certificate, service center some time send RFE about I-140 approval copy or I-140 receipt, if I-140 is pending then attach the copy of filing receipt and take the print out by punching I-140 # from BCIS ONLINE status for I-140 and attach that also

For EAD renewal, how should we take care of FP?
E-filers go to local office to get photo and FP. If we mail the application, do we need to attach FP also?
