However, a Philippine passport in a different name might pose a problem, no? Hopefully you can show the birth certificate, and also other evidence of how your name has changed (but not US documents), and they can issue you a passport in your new name. Would this work?
 
I will go to National Statistics Office try to ammend my last name in my local birth certificate to my father's surname i think that's the easiest way so that i can get my philippine passport. Im planning to go to US this may or june, the immigration told me if i can get a philippine passport i don't need to pay such fine or penalties.

yap im gonna show them my admission of paternity i think that would be enough.
 
Since you've been using it ever since then, and it's on your CRBA, for all practical purposes it is your legal name now (common-law name change).

Just make sure you don't use the birth certificate from the Philippines for anything in the US, as the name difference may create complications. If you're ever asked to show your birth certificate when applying for something, show the CRBA and it will be accepted as the equivalent of a birth certificate. I don't think you'll have to show the actual birth certificate for anything in the US, unless you're running for President!
