How to apply for Adavanced Parole?

Following Documnets - Jaxen please confirm
I searched this forum and have found following,
1. Form I-131 (filled in black ink).
2. Two ADIT photos(write A# on back)
3. Cashier's check or Money Order of $110/. (No personal checks since March 01, 2003 unless applying in person).
4. Copy of I-485 Receipt.
5. A cover letter requesting AP.
6. Copy of previous AP. (some say it is not necessary)
7. Copy of I-94. (Some say it is not necessary).
8. Some have included copy of Passport which I find unnecessary.
Please correct, if I am wrong.

No I-485 receipt number, what to do?
I prepared the required set of documnents as per the list in previous thread, one for myself and one for my daughter. But I do not have a copy of I-485 receipt for my daughter. What do I do?
I have a copy of her previous APs and EAD. (EAD was issued even though she is below 14 years of age!).
Please advise.

2) I am not planning to travel and dont have immediate plan what should i put in "PART 3"
a)Date of intended departure
b)Expected length of trip
3) Should I use Black pen to fill the form.
4) $110 fees??payable to whom (I am in Sanjose, CA)

put some date and duration- it is not binding.

Excuse me for ignorance
1) $110 should be payable to Bureau of Citizenship and Immigration Services ...since my case is pending there???
2) I have to send one application for me and 1 for my wife, so it should be 2 different envelope or 2 separate envelope in a big envelope good??

I had also included a letter to BCIS giving the reason for requesting AP. It is a std letter stating that you may require to make one or more trips to your home country to visit parents /relative etc. Also be sure to check the AP for multiple visits
