Help: H1b Application pending for over 3 years

My Initial application has been pending for 3 years [since OCT 2004], do I need to file for extension like regular H1b holders, or is that not applicable !? If not, is it even possible to stay pending indefinitely !?

Would the starting date be the day of the approval or the original one from the application of 3 years ago !

What is your status now. Do you have any I 94? If yes what is its expiry date?
