You have to apply for your dependents separately meaning separate forms (DS-156/DS-157) and separate application fee.

My wife and I are both in the US currently on J1/J2 so I guess my wife would need a I539 to adjust her status?

Thanks - the I539 form says to use that form for dependants of H1 - is that not correct? would she not be adjusting from J2 to H4?

I-539 is filed for CHANGE OF STATUS or Extension of H4
dependents of H1 status holder can file Change of status with form I-539
