Form 445 Signature, please help

Hi, on the second page of Form 445, in which we are supposed to answer a series of questions, do we sign the form using our normal signature (mine is illegible) or print our name?

Is there anything else I need to know about this form other than answering NO to all questions and sign it?

does one sign this in front of them?

I somehow thought one needed the cursive signature on this, like the photos, and nat. certificate itself...will there be a problem if one did the cursive/formal one?

also, it says "at" i guess that'll be "Durham, NC" in my case???
 
I filled it out and signed it with my regular signature before entering the DO. The IO at front desk looked at it real quick and told me to proceed to oath room.
 
I forgot how I signed, probably my usual signature. I don't think it matters terribly. In my case, showing the GC, my person and the oath letter was done concurrently. I think you can sign it the morning of your ceremony, or the night before. If something changes in the hour between your signature and delivering it at the ceremony I guess one could make amendments to the information.
By the way, I think I remember seeing some person at my ceremony that incorrectly thought that he/she needed to fill out the arrest information not for the period between interview and oath, but for all the previous years. There was an IO trying to straighten out the little "mess" :)
