DS-156 form questions

Hi,

I came to the US as a student (F-1).. then started working immediately after university. I just renewed my H-1B for the 2nd time but I never got my H-1B stamp and will have my interview next week in Vancouver. I also have an approved I-140 application. Anyway, I have some questions regarding the DS-156 application form

Q15. Home Address - Do I enter my current US address or my address from my country of residence (Philippines)

Q26. How long do you intend to say in the US? - Do I enter the expiration date of my H-1B?

Q30. Have you ever been issued a US visa? - Again, I never got an H-1B stamp.. do I write H-1B as the type of visa or do I enter F-1?

Q36. Has anyone ever filed an immigrant visa petition? I have an approved I-140 application so I'm assuming I will select Yes and write the name of my employer as the Petitioner.

Anyway please let me know if I got this correct. Thanks!

thanks for the reply! but i just wanted to clarify Q30. When it asks for the type of visa that's been issued to me.. do I enter F1 or H1? I'm just not sure because I never got my H1 visa stamp.

if in PAST you got F1 Visa then write F1 if in past you got H1 visa write H1 if in Past you got O-1 write O-1 if you got B1 in past write B1 if got B2 write B2...............
 
sorry if i'm being thick headed here.. :(
My H1 petition has been approved but I never got an H1B stamp, does that mean I was issued an H1 visa?
