A friend of mine went to the embassy ang got denied for a visitor visa. What is the procedure to appeal to decision???

does he need to create a new file and reapply??

There is no appeal process. Your friend is more than welcome to re-apply.

did reapplied...denied again under 254...I guess not more usa for him in the short term...

I assume you mean 214.

It seems your friend does not have strong enough ties to his country of residence in order to overcome "immigrant intent".
