Your INS appionted doctor should know what forms to file and how to do that. They will give you sealed and stamped envelope, which you are not suppose to open.

I called immigration yesterday and got transfered to IO - very lucky. He told me to use my expired national passport, because they would know that as asylee I can not get a valid one. He also told me to send all information at the address at the corner of the yellow letter.
