Do I need to submit my original I-94 or copy of it?
What reason should I write in the letter to USCIS, as I don't have any assistanship.

You can have two H1s, So let the employer B also file for your H1. If approved you can directly start working with employer B otherwise option 1 will be there (when approved with employer A).

Similar is the case with my friend. He applied for H1B at chennai consulate in Jan 2004. After a short interview they asked to deposit $85 fee and took his fingerprints for security check. I guess that they do this if you make a name check hit in NCIC-III FBI database...
