We received an online message yesterday Nov 15 on my wife's approval. About 4 weeks after USCIS received the test of her medicals (the RFE response)

We did a complete medical for my wife. And sent the doctor's sealed envelope directly back to INS. Your doctor will have to include the RFE letter in the sealed envelop as well. We did not send any other docs to INS just the doctor's sealed envelope.

Vaccines
We were expecting a baby during our 1st medical so my wife did not take all the vaccines. The reply to the RFE was basically the sealed medical results as requested for by USCIS. I mailed the test results back to USCIS myself.
Just got stamped
I got stamped without submitting AC-21 papers. The officer just asked me where my petitioning company is located and I told him without providing any other info. AC-21 applies to pending 485 and if approved, you do not need to complicate your case. Don't ask don't tell.
Not Sending
I changed jobs using AC21 and did not inform USCIS. My case was approved last month and I have my passport stamped. I do NOT intend to inform USCIS of the job change.
Possibly Medical
From the info above, my wife's RFE may be for medicals as we were expecting our baby at the time of the medical exam and she did not get all the vaccines I got.

You can
You can use AC-21 if your 140 has been approved, your 485 pending for more than 180 days and the new job is similar to that approved at labor cert.

Please does any one know the typical time between a principal's 485 approval and that of the spouse. My 485 was approved some days back and no news on my wife's. I called USCIS and was informed that hers will be processed in its own time.

You can
AP has nothing to do with AC21. You can apply for AP or EAD as long as you have a valid 485 pending. Once you change jobs you'll need to travel with AP as you are no longer maintaining H1 status unless you are transferring your H1 to the new employer.
