When I am trying to apply to renewal of OCI under miscellaneous services and enter passport #, OCI # and Date of birth, I get message stating :No record found for the information provided. I triple checked to make sure all info is correct. Please see the screenshot.

there is no tax form in Pk-4. But you can call IRS and get Tax Transcripts from last 3 years and keep them with you for just in case scenario. greenMD

If you have already paid 335 $ per person, then you do not have to pay anything at mumbai. Make sure to carry receipt of fee paid to NVC with you. GMD

In PK-4 intruction it is said that Birth affadivait should be signed by FIRST CLASS MAGISTRATE. What is difference between PUBLIC NOTARY and FIRST CLASS MAGISTRATE .

my pk -4 was send on march 3/4. i will be leaving for mumbai on 24 march. even if you do not get p-4 in time you can pick it up at consulate window before your med exam day.All you need is interview letter from that pack. Other you can get it from internet. So do not be nervous...

Your NVC automated message should tell you name of consulate. After that call / email that consulate to find date early or check their website after 15 of march.
