Ok, this is what happened in my case.
Changed address 08/2003. FP2 arrived at old address, was resent to new address by INS.
Approval letter and kids Cards arrived at the new address.
As long as the copy from the municipal is in English, you are fine. I had a similar RFE for my son. Otherwise you have to get it translated and notarized.
You could either trace back the medical documentaion or pay the extra $$$ and get the medical redone. I would get it redone.

I would recommend to use AC21. That way you can have peace of mind. You may not even get an RFE, but even if you get it, an RFE for EVL is not a big deal.
