Your letter formats seems to be alright. Also see many discussions in the thread "Birth Certificate..." by Optimist99 in this forum.
You do not need to execute in Stamped Paper. Plain A4 sheets will do in US.

This is a standard RFE for anyone who has not registered their birth at the time of brith and got the BC later on. Or if they don't have a BC at all.
The type of text in the affidavit may vary case to case.
Probably in your case, both your father and mother may have to issue two affidavits...

I didn't call INS or Automated Voice Message System even once. And I've never talked to an IIO.
All I did was... monitor the INS Case Online Website.
