Aorry, don't know My NOA didn't tell me to go to a specific ASC location. I doubt that it matters where you have them done though. It may be worth to just call the National Customer Service Center at 1-800-375-5283 to ask and schedule an appointment.

Go to Salisbury the Salisbury ASC on the Eastern Shore has plenty of appointments available. I could schedule for NEXT day!
