Once again, I hope I'm not giving you the wrong advice and you may wish to clarify everything I say with others who know better. But this is what I have done with my KCC forms.
I sent the downloadable forms back to the KCC, not the original files they sent to me.

I used the autofill option on these forms as my handwriting is atrocious.
Please don't forget to write your case number on the top right corner of EVERY PAGE of your forms.

I see that you sent your forms back from within the U.S., I probably would have also used this option if mailing the forms back domestically if that service offered proof of delivery.
However, I mailed mine back from Australia so I used Australia Post's 'Express Courier International' service...
