You can use your new lawyer to represent from now on and make sure that you fill out G28 form and send it along with Ac21 papers. ps: $1000 for Ac21 is quite a bit and you can send the papers by your self but since your new employer's lawyer is not charging any money that should be fine.

Yes you are better of staying on H1 untill I485 is approved unless you have to work on EAD. You can apply for H1 extension ( 7 th year) that should not be an issue.Transfer your H1 and wait untill your I140 is approved if you want to use AC21(by that time you should pass 180 days of I485 filing)

I assume you have taken medicals not an exam .so if you have your medicals done on 03/23/2004 then they are valid till 03/23/2005. JK is right once they are submitted they are valid untill your I485 is adjucated. They usaually cost between 150 to 250 per person..
