Assuming you are US citizen of Indian Origin, you may want to apply OCI /PIO card. Once you have the card, you should be okay (you might then have to apply for PAN card).
This is just my opinion. you may want to check with a lawyer/CA in India.

Yes, certainly. You can take one appointment (in the appointment, mention you will have one dependent along with you for stamping). You will have to fill out seperate DS-156 forms for yourself and your wife though.

I think they have stopped sending letters since 2006. You need to take printout from the website where you took your appointment. There is a button just next to all the forms that you have to fill out and print (DS-156 etc...).
