Which I-94 to deposit leaving the States after H1 Transfer?

I have one (white) I-94 given to me when I last entered US.

I have one I-94 attached to the base of my approved I-797!!
Im leaving for India for revalidation of my H1B..

which I-94 do I deposit when leaving USA? both of the above?

do I need the I-94 at the bottom for H1 Revalidation of my I797?

You need to surrender ALL I-94
Staple both I-94 and give it to airline counter

*Make the copy of approval notice that has I-94 attached (front and back ) and keep the copy for your records.
