Urgent Help!!! Do I need to submit I-94?

This weekend I am planning to go to India. My original I-94 (at port of entry) expired on June, 2000 and I got a H1 extension.

Should I only submit the original I-94 (white card at port of entry) or should I submit both the original I94 and the one that comes along with my I-797 extension (right bottom portion of I-797?

But the one that came with the notice is the latest, that should be enough. Do not foget to get the stamp in your passport.

I got a new visa stamp from Dept of State few months back. Only my I-94 was expired (white card). In the port of entry all I need to show the left bottom portion of I-797, right?
