Self EAD Renewal application

I was going to apply for my EAD Renewal self this week.

I have the I-765 form filled correctly.

I have the old EAD copies included

I have the I-485 receipt included in the packet.

I have a covering letter (just to make things indexed) on the top

HOW do I attach the photos? DO I write my name on the back of the photos, and if yes with a pen or a pencil. Also 2 ADIT style are OK right.

Not to mention the $120 check

pen. Name, A# and I485 receipt number, have to be ADIT style. Write the A# and 485 receipt number in check, cover letter in bold (like a big foot note).
Put all these in an envelope and write "Application for EAD"
 
1) For the $120 check, is it ok to send a personal check and payable to whom ?

2) For the I485 receipt, is photo-copy one OK ?

3) Is there any place I can get detailed information in how to renewal EAD ?

2. What other copy can you produce?
