Self addresed fedex envelope. help

Since the Passport after revalidation are going come from Washington DC. But we have only ST LOUIS ADDRESS. WHAT should be FROM address in return envelope. Will it be

The FROM FedEx envelope that you will send for RETORN of your passport should have:

FROM
Your Name
Your address
your phone #


TO,
Your Name
Your address

your phone #


**** provide the credit card # that is valid for 4-5 months

"From Address" must be your address. Please see following on the state department site.
