Seeking H4 Info~~~

Hi All! Have few questions regarding H4 Visa.
I thankfully appreciate all your answers/feeds.

1. How important is the Bank Balance Statement (i'm more concerned with funds)?
2. What things I should be chary of (any tips)?
3. Please correct my checklist for H4:
a.) My H1 visa along with 727 and I-129
b.) Letter from employer stating status
c.) Pay stubs
d.) w2
e.) Marriage snaps along with certificate of proof
f.) Bank Statement

PS: I am goin 2 Calcutta Consulate for Visa
 
Bank statement is not very important for H4, W-2 can be used as proof that you got paid regularly.
Corrections to the list
a) 797 not 727 and LCA copy
b) Letter from employer , stating status, salary etc. along with para requesting H4 ...
 
Changing your wife last name on the passport with your's is recommended. We just got it ours done in mumbai, name change was one of the questions asked.
 
Name Change

Can you tell me what happened when you applied for H4. Did consulate gave h4 only after the name was changed or it was just a question asked in the interview. I am planning to get my visa stamped from Delhi Consulate and I have a different last name from my husband.

Well, i don't know if it's a requirement, but we had it changed before the interview. If you have time, it's better to change the name.

One of my friend got rejected in madras for the same reason, but
it wasn't a requirement in madars(chennai).

My guess would be changing the name.
