Returned I-693 sealed envelope

GC Status :I 485 application is in process (I 140 approved)

A sealed Medical examination Envelope returned enclosing a letter stating that "The medical Examination report, Form I 693 submitted with your application was not signed by you and is therefore not acceptable". Also the directions were given to "Take this sealed envelope to the civil suregon who conducted the medical examination and sign on I 693 form in presence of civil surgeon and return the sealed envlope"

My situation: Currenly I'm moved out from the state where I went for medical examination. This query is for my son who's age is 4 years old(he cannot sign on the form)

My questions : Is it adviceble to attend again for a medical examination with any local approved civil surgeon and submit a fresh I693 ignoring to visit the same doctor who conducted the medical examination ? Please suggest
 
Yes, it is advisable to go ahead and get a fresh medical examination done.
Please ensure that the doctor's office includes a cover letter confirming that the examination was for the 4 year old child and that you are his legal guardian.

It would be ok for you to include a cover letter writing down the specifics of your case and about your child, and include it with the sealed envelope that you receive from the doctor's office.

It sounds fairly ridiculous that they have not seen this and have rejected it based on the data for a 4 yr old kid, but then, thats them...

Go to the same or different civil surgeon, sign the old forms or new one are the same. Your decisin will affect how much fees you pay.The USCIS needs the forms (old or new) signed by you (not your kid for sure) and sealed in new envelope from any approved civil surgeon.
