renew EAD and AP

help on 765 & 131

Hello Ginnu,

- My employer filed my 485 from Vermont, but I live in California now. To which center shud I send my EAD & AP extension documents?

-Form 765: 15. Current Immigration Status
What shud I fill in here?

- Form 131: 3. Class of Admission
What shud I fill in here?

-Form 131:part 7. For how many times do you intend to use this document?
If say two or three times, will it have a negative impact on the approval?

Hi Ginnu,
One more silly question:
The 765 document for EAD extension that I downloaded is 13 pages. Should I send all these 13 pages to USCIS or just the last "fillable" page?
The AP application 131 is also 10 pages long. Should I send all 10 pages are just the fillable last 3 pages?
