Questions Regarding DS-156,

Hi,

I am on the verge of applying to the St Louis center to get my the visa stamp on my passport revalidated. My earlier Visa expired Nov 1, 2003.

I need some clarifications regarding the Form DS-156

Question 21: Present occupation.

Question 25:Name and address of the person in US you will be staying with or visiting for tourism or business. What do I answer here since I am in US on a H1B status. What do I answer on the forms of my wife and children

Question 27: What is the purpose of your trip. What should be answer in my form and in the form of my wife/children

Question 30: Have you ever been issues a US visa: I have been issued two visas so far, one in Mumbai (the 1st H1B visa) and then a revalidation in Vancouver. Which visa should I state in the question when and where

Question 34 : Names and relation of persons travelling with you

Question 35:Has your US visa ever been cancelled or revoked: My first visa was marked "Cancelled without prejudice" when I got a new stamp at Vancouver. So should I answer yes or no to this question

Question 37:Are any of the following persons in the US: Wife/hunsband, Son/daughter etc. My wife and children and with me, what should I state as their status, and in my wife's form what should be the status of the husband and in childrens' form what should be the status of brother, mother and father

Write your present H1b occupation (engineer, programmer, etc.)

Question 25:Name and address of the person in US you will be staying with or visiting for tourism or business. What do I answer here since I am in US on a H1B status. What do I answer on the forms of my wife and children
For you write your employer's name and address. For wife and children write your name and address.

Question 27: What is the purpose of your trip. What should be answer in my form and in the form of my wife/children
For you, authorized employment under H1. For wife/kids living with you as family.

State yes, include both visa issue dates and places.

If your wife & kids are travelling with you to US, include their names.

Answer No because the visa cancelled were already expired and you were issued a new one.

Question 37:Are any of the following persons in the US: Wife/hunsband, Son/daughter etc. My wife and children and with me, what should I state as their status, and in my wife's form what should be the status of the husband and in childrens' form what should be the status of brother, mother and father
Why is this question so difficult for you to answer??

I am still confused about Question 37. I am not clear what should I write the status of my wife and children and what status my children should write regarding my wife and each other.

----- if your wife or child are on H4in US then write :H4 status

Father=in H1
Mother=in H4
Child=in H4

Choose appropriately based on who is filling out the form.
I still don't understand, what is the confusion?

Hi,

I do not know, Is it right place to post this question. But I am also similar confusion to fill DS-156 for my parents and in-laws. They are going for visitor visa at Chennai.

I and my wife are on EAD/AP. My H1B expired (6 years). I did not apply for 7 th year extn.

Q37. Are Any of the following persons in the U.S., or Do They have U.S. Legal Permanent Residence or U.S. Citizenship?

what are the status????
