Question related DS 156

Hi,
I was on F1 status but now I am on H1B visa. I have been working on H1B from past 4 months.
I have an VISA stamping appointment at Vancouver, Canada on 9th of March. I was filling DS 156 form and I have following doubts ?

1) Name and Telephone Numbers of Person in U.S. Who You Will Be Staying With or Visiting for Tourism or Business ?

Now I am already staying in US in an apartmenm so what kind of answer do I give here

2) How Long Do You Intend To Stay in The U.S.?
Now I have H1B till 2008. So should I write till 2008 ?

3) What is purpose of your trip ?
Now what kind of question is this ? I am working here on H1B so what do I write here.

4)Who Will Pay For Your Trip?

Is it ok to say myself.

5) 29. Have You Ever Been in The U.S.?
Yes No WHEN?
(Most Recent) day month year
FOR HOW LONG?

Now I am completely perplexed at this question. What do I write here. I came to united states as a student in dec 2002. Then went back to India once and then again came and then again here from 4 months.
So what do I write here ?

I think it says "National Identification Number (if applicable)" that means if you don't have anything to write put NA, thats what I did.

Your company's HR is fine.

Not sure. you may claim you are on vacation and you are paying on your own.

If it says "most recent", then it should be most recent stay which is current one.

22. When Do You Intend To Arrive In The U.S.?
(Provide specific date if known)

I am gojng to mexico, what should I write here ??? or should I leave blank?

I already submiteed one time, I see made mistake, is there any problem if I submit new one with fresh bar code?

In case of VFS website, you can login change & print the modify DS 156 and it retains the same bar code.

But, for mexico I don't think that website where you make appointments generates the DS 156. So u can fill & print from the http://evisaforms.state.gov/

and yet this one generates a different barcode every time u create a new DS 156. Its fine. Worst case call up the nvars folks and ask clarificaion.

I do NOT know what should I do.....I think I should do new one with new bar code and forget about this.......or let it be I can explain them I did a mistake....shoot very confusing...

Why can't you make corrections in the same form with Pen? Are we not allowed to do that, I think on the top of the form it says type or print so it should be fine if you use pen. You should just call the consulate and ask them what can you do in this case and follow their instructions.
