question about filling I-131 with I-485

I'm filling I-131 with I-485, and need help on some of questions.

1. Two photo I submit, should I submit the same format as I-485(front face) or I should submit the format of showing the right side of my face?

2. I was J-1, but got waiver later. Is my situation a problem?
Because I find that they are not gonna issue AP to follow condition.
You held J-1 nonimmigrant status and are subject to
the two-year foreign residence requirement as a

2) not a problem, since you got a waiver.

What does "Because I find that they are not gonna issue AP to follow condition." mean? I tried to decipher it....
