Hi, I just noticed this in the RFI letter I received. In the #2, Supplemental form to I-693, it says "Also, submmit a copy of any documentation that identifies the physician completing the supplement as a civil surgeon." If I understand it correctly, in addition to I-693 and the supplement, I'll also need to submit something to prove the physician is a civil surgeon, such as a copy of his liscense? Am I right? Thanks.

Thanks for your reply. When I went to the exam (haven't picked up the forms yet), the doctor already told me what he was going to seal in the envelop: just the I-693 and the supplement, with his name, address and signature. Should I ask him for a document to prove he's a civil surgeon?

I think Doctor's stamp and signature is proof enough. Obviously they are not going to give you a copy of their diploma or whatever. As long as, you pick the civil surgeon from the USCIS approved list and that doctor signs and stamp your results, you should be fine. Don't offense your doctor by asking him to prove his/her credentials. If you piss him off, he might mix some Diarrhea medicine in your MMR shot and you'll be spending all night in your bathroom cursing yourslef, why in the pete's name did I ask that question from my doctor??
