Proving doctor's credential

Hi, I just noticed this in the RFI letter I received. In the #2, Supplemental form to I-693, it says "Also, submmit a copy of any documentation that identifies the physician completing the supplement as a civil surgeon." If I understand it correctly, in addition to I-693 and the supplement, I'll also need to submit something to prove the physician is a civil surgeon, such as a copy of his liscense? Am I right? Thanks.
 
Don't worry the Doctor will give you a sealed envelope with your results and I am sure he or she will include the credintials in that very envelope.
 
Thanks for your reply. When I went to the exam (haven't picked up the forms yet), the doctor already told me what he was going to seal in the envelop: just the I-693 and the supplement, with his name, address and signature. Should I ask him for a document to prove he's a civil surgeon?
 
If the doctor is on the list on USCIS wesite, than I suppose they should already know his credentials.

Karina, I agree with you. He's on the list. I'll ask him anyway. If he doesn' want to give me anything, then I'll assume CIS will look him up in their own list. Thanks.
 
Credentials

I think Doctor's stamp and signature is proof enough. Obviously they are not going to give you a copy of their diploma or whatever. As long as, you pick the civil surgeon from the USCIS approved list and that doctor signs and stamp your results, you should be fine. Don't offense your doctor by asking him to prove his/her credentials. If you piss him off, he might mix some Diarrhea medicine in your MMR shot and you'll be spending all night in your bathroom cursing yourslef, why in the pete's name did I ask that question from my doctor?? :D : ;) :p
And "No", I did not ask my doctor his credential, if that's what you are thinking, you devil you :rolleyes:

PM, I am always on the lookout for your and MK's posts. They are so funny yet true and to the point. Not offensive at all. I wonder how you come up with these ??? And I just cant help myself butting in...usually I stay away.. :D :cool:

I am LMAO and thinking, what if one of the forum member asks the Doctor for his credentials, right before the Doc is ready to perform test of "HERNIA". :D :D :D :D :D :D :D
