Problem with I-94 when filling out form I-485 and I-130

Last year (July 2008) I went to Canada for vacation for 4 days in student visa (F-1) and came back to US to continue school. I just realized now that I didn't get the new form I-94 from re-entering the US. I just got a stamp showing the date i entered the US which is JULY 24, 2008 and "D/S" expiration date on one of my passport page.

So the form I-94 that i have in my passport right now is from the trip i went back to visit my home country, Thailand, in Jan 08. On that I-94, i got the stamp on the form, showing the date of my re-entry which was JAN 24, 2008 with the "D/S" expiration date.

The problem is now i'm applying for Adjust of Status which i need to fill out form I-485 and I-130. On form I-130, they ask me to fill out;
- no# of I-94
- Date arrived
- Date authorized stay will expire, as shown on form I-94
=> I'm not sure that "date arrived", should i put the date arrived on I-94 or date of my recent entry!!??? ( if i put date of last entry, it wouldn't consistent with the I-94 no# which got from re-entry from Thailand).
=> Date authorized stay, should i write down "D/S" as it appeared on I-94??!!

On form I-485, they ask me to fill out;
- Date of last arrival
- no# of I-94
=> date of last arrival (from trip to canada) is not consistent with the no# of I-94 (from trip to Thailand)


Have anyone ever got this problem same as me before? What should i do? Thank you in advances for your help.
 
I used Date of Last entry for everything .. Also teh I 94# remains same for every I 94 u have or received. Yes your latest I 94 is valid till u finish ur education d/s stands for duration of stay usally students get that .. YEah D/S is what you have to use ..
 
Under certain conditions, they don't give you a new I-94 after taking a short trip to Canada or Mexico. So don't worry that the date the I-94 was originally issued is different from your last date of entry. They know the rules, write the date of your last entry and you'll be OK.

I have one more question regarding port of entry that i need to fill in the form I-485. My last trip was to Canada. The stamp in my passport when i re-enter the US shown "CHM" as the port of entry. What do i need to fill in as the "Place of last entry into the US (city/state)" in form I-485? I'm not sure if the state i enterd was NY or Vermont.. Thanks again for your help !
 
I'm not quite sure whether it was New York or Vermont. I took the China bus tour and they changed the schdule..but i'm a little bit sure that it was New york. Whst should i do? I'm trying to look at US border security website, but there is nowhere to tell me what does the CHM stand for.

I had a similar situation for my daughter, what I did was to file 102, reason, I94 was not issued at port of entry, then evidence i submitted her 1-20 that was stamped. On application I simply put None. I filed 102 together with the rest of the forms. I received a new one in less than 3 months. Am glad I did cos the officer at the interview asked for it, and I didn't have to do any explanations about not being issued one. I'd hate to have anything hold my case. Just personal experience.

Was this after a short trip (under 30 days) to Mexico or Canada? If yes, the lack of giving a new I-94 probably was deliberate and those steps to get another one were unnecessary. Unless the I-94 was surrendered when leaving the US, in which case you'd have to take steps to get a new one anyway.
